Most likely it's either because it's not signed OR the consent form does not contain a reference to Education Verification / Degree Verification. It must be clear on the form that the consent is given for this purpose OR the form itself is eligible.
*UPDATE: Sherbrooke will only accept WET signatures. They will reject any typed, copy/pasted or electronic signature*
